Key Takeaways
- Penile enlargement with dermal filler is primarily about girth, not erect length.
- It’s non-surgical: no incisions, most patients return to normal activity the same day.
- Initial results typically last 12–18 months; maintenance treatments keep results going.
- It does not treat erectile dysfunction, and results are not permanent — be wary of anyone promising otherwise.
Let’s cut through the noise.
If you’ve been researching penis enlargement or penile enhancement, you’ve probably seen some pretty big claims. Pills that promise inches. Devices that claim dramatic results. Treatments that sound almost too good to be true.
I’m a board-certified surgeon, and I believe you should know exactly what a procedure can and can’t do before deciding whether it’s right for you.
So let’s start with the most important thing:
Penile enlargement with dermal filler is primarily about increasing girth, not length.
How Does Penile Girth Enhancement Work?
During the procedure, dermal filler is carefully placed beneath the skin of the penis to add volume around the shaft.
The goal is a thicker, fuller appearance while maintaining a natural look that follows your existing anatomy. Many patients also notice they show more in the resting (flaccid) state — but to be clear, that’s different from erect length, which filler does not significantly change.
This is a non-surgical procedure. There are no incisions or stitches, and most patients are able to return to their normal daily activities the same day.

Does Penis Enlargement Hurt?
This is one of the most common questions I get.
I use a nerve block to numb the treatment area during the procedure, which keeps discomfort to a minimum. Most patients are surprised by how easy the experience is.
Afterward, some swelling, bruising, or tenderness can occur. Small areas of unevenness or lumps are possible but uncommon as the filler settles — and any imperfections are easily correctable. This is one reason why proper technique, medical oversight, and follow-up care matter so much.
You’ll need to avoid sexual activity for a period of time after treatment while everything heals and settles. We’ll go through your specific aftercare instructions during your consultation.
What Penile Enlargement Can’t Do
This part is just as important as talking about what the procedure can do.
Dermal filler does not significantly increase erectile length. As noted above, some men do show more at rest — flaccid appearance and erect length are two different things.
It also isn’t a treatment for erectile dysfunction. ED can have several different causes and requires its own evaluation and treatment plan, which I can provide — this typically includes testosterone replacement therapy and a VSEL/stem cell injection procedure.

And results from dermal filler aren’t permanent. How long results last varies with the filler used, your body, and other individual factors — but in general, initial results last 12–18 months, and maintenance treatments are needed to keep them. VSEL injection results are more lasting, because that’s truly a regenerative procedure.
That’s why I’m cautious when I see anyone promising permanent or guaranteed results from a single treatment.

What’s Considered a Normal Penis Size?
One thing I tell patients all the time: many of the men who come to see me are already well within the normal range.
A large systematic review (Veale et al., BJU International, 2015) found the average erect penis to be approximately 5.2 inches in length and 4.6 inches in circumference.
For many men considering girth enhancement, there isn’t actually a medical problem that needs to be “fixed.” It’s an elective, personal decision. Some men simply prefer a fuller appearance or feel an enhancement would improve their confidence — and there’s no medical “need” to have this procedure simply because you fall within the average range.
How Much Does Penile Enlargement Cost?
Cost depends on the type and amount of filler needed to reach your goal, whether VSEL/stem cell treatment is added, and your maintenance plan over time. Because every anatomy and goal is different, I quote exact pricing during consultation — but here’s what drives it: the product used, the volume required for a natural-looking result, and how often you choose maintenance treatments. Be skeptical of bargain pricing; this is a procedure where injector skill and proper medical oversight directly affect both safety and outcome.
If You’re Considering Penile Enhancement
Ask questions. Understand what the treatment can realistically accomplish. Understand the risks, recovery, and maintenance involved. And make sure your procedure is performed in an appropriate medical setting by a qualified medical professional who understands the anatomy and knows how to manage potential complications.
The goal shouldn’t be unrealistic promises. It should be a natural-looking result, appropriate medical care, and honest expectations from the beginning.

Frequently Asked Questions
Does penile enlargement increase length?
Dermal filler primarily increases girth, not erect length. Some men notice they show more in the flaccid state, but erect length does not change significantly.
Does penis enlargement hurt?
A nerve block numbs the area during the procedure, so most patients report minimal discomfort. Expect some swelling, bruising, or tenderness afterward, which resolves as the filler settles.
How long do penile filler results last?
Initial results typically last 12–18 months depending on the filler and your body. Maintenance treatments are needed to sustain results. VSEL/stem cell results are more lasting because the procedure is regenerative.
Is penile enlargement permanent?
No. Be cautious of anyone promising permanent or guaranteed results from a single filler treatment.
Can dermal filler treat erectile dysfunction?
No. ED has many possible causes and needs its own evaluation and treatment plan, which may include testosterone therapy or regenerative options. For more information about VSEL treatment for erectile and sexual performance, visit Energy MD’s VSEL treatment page.
